Has anyone applied for early release of super on medical/dental grounds? Were you successful? Any tips? The forms are a bit daunting. ....

Yes I got funds released from my Husbands super for dental treatment. The forms are a little daunting but fairly simple and straight forward. Just be sure to get quotes for everything you need. If it is surgery make sure you have surgeon, anesthetic, hospital room all on quotes. Along with a letter from your employer outlining the time off required and how much of that time is paid leave and unpaid so you can possibly claim the unpaid leave aswell. If its teeth and you are having dentures put in make sure you have a quote for that to along with the first visit after to have them adjusted.
I also had to get a letter from my gp to say that I needed the surgery for my teeth to stop severe infections. Aswell as a letter from a dental surgeon to verify that this was needed asap and that the public wait list was extremely long.
There are certain things you need to prove (constant pain, health issues without the treatment, that waiting for the treatment within a public system would cause more problems) otherwise you may be denied.
