How often is she changing her tampon? I know some people like to leave the maximum time, but I find on my heavy days I need to change quite often, every few hours at least.
She could also try period pants, or a cup.
Our GP has suggested birth control in an effort to hopefully regulate her periods and lighten them. As you find heavier periods usually bring with it stronger pms symptoms too.
You're not doing anything wrong. I got this as a teenager and then late 30s it became that bad again too. I used to use a super tampon and heavy flow pad and hope that would last from 1 break to the next at school (80 to 120 minutes depending on what part of the day it was). It was really hard to manage. I would keep a jumper in my bag to wrap around my waist if I was concerned that it hadn't been enough.
I used to get extreme pain as well, so I went on a very low hormone pill. It still took a few years to settle, but it meant that I could skip my period a lot of the time. The one I went on isn't recommended anymore though as it had mental health side effects and blood clotting side effects for some people. Talk to your GP if you are thinking along these lines.
Like I said, I get this issue again now as well. I find that a very heavy flow period undie is a much better back-up to a super tampon and means I don't have to worry about not making it through a meeting at work. They hold a lot more. That said, if I try a brand that isn't ModiBodi, they're useless and can't cope. Also, period undies break down and smell despite washing them if you don't soak them in vinegar semi frequently. I find this is a much bigger issue if they've been filled to their capacity even once.
As a side note, if pain is an issue, it can be worth asking whether PCOS/Endo is worth checking for And with a heavy flow, it's also worth asking if your GP would like to keep an eye on iron levels.
You're not doing anything wrong. I've had heavy periods on and off since I got my period over 20 years ago. Maybe take her to the dr just to rule out things like PCOS and Endo

I would definitely take her to a doctor and get her periods checked but in the mean time may try some period underwear. Modibodi is great and has different size undies for different flows. They also have swimwear
Has she tried ponstan?
You can get it over the counter
It lessens the flow
My daughter takes it as her flow was so heavy she was iron deficient
