Has anyone got a child’s passport under special circumstances? (B9 form) could you please tell me the outcome & what was the process like? Any information would be greatly appreciated. My daughters “sperm donor” hasn’t been in her life since she was born. She is now 12 years old. We are planning a holiday overseas at the end of the year but have been informed that “sperm donor” would need to sign as well. I have no contact with him & have no desire to do so. Please help. I am incredibly upset that he hasn’t been in her life for 12 years yet his rights get to dictate my life.

You can fill out the form, then it goes to a judge :) no need to contact Dad if you can prove he has no contact
